Captions tell the respondent what information needs to be filled in within a particular field. Whenever possible, the caption should be placed outside the box - above, or to the left - to eliminate interference and to better call out the information being recognized. This identifies the information to be filled in and helps guide the reader. Captions should NOT be used under a line or box, where they can be mistaken for the caption below it.
Pre-printing special characters required for certain fields can also improve data quality. If left to the discretion of the respondent, the information may not be completed in the required format. For example, it is not uncommon for respondents to leave out an area code completely when filling in phone numbers. When requesting this information, fill in the parentheses and dashes to separate the area code and seven-digit number.
